Please bring your completed consent form, insurance card(s) and proof of Illinois residency/employment to your appointment.

Single dose J & J COVID-19 vaccines will be given Monday, April 12 from 2-7 p.m. at the Olney City Park Community Building. Registration is open to individuals 18 and older who live or work in Illinois.
